This research employed a Research and Development (R&D) design with two objectives: 1) to develop a multi-agent chatbot to support holistic health care among older adults, and 2) to evaluate the system’s performance and users’ satisfaction. The participants were 81 adults aged 60 years and over residing in Phetchabun Municipality who regularly used smartphones and the LINE application. Participants were recruited through convenience sampling and used the developed system in a real-world setting for four weeks. The chatbot was developed on the LINE Official Account platform by integrating a large language model with workflow automation. The system consisted of five major components: an Intent Classifier, a Risk Analyzer, an emergency Safety Gate, five specialized agents covering physical, mental, social, wellness and daily living, and spiritual health dimensions, and a Memory Database for storing user profiles and conversation histories. The content validity and appropriateness of 51 health-related questions were evaluated by three health experts. In the first round, 45 items (88.24%) met the Index of Item-Objective Congruence (IOC) criterion of ≥ 0.50. After revisions based on expert recommendations, all items passed the criterion, with an overall mean IOC of 0.83. The system performance, as evaluated by three information technology experts, was rated at the highest level overall (Mean = 4.70, S.D. = 0.45). The highest-rated dimension was functional requirements (Mean = 4.83), while the accuracy of emergency alerts received a mean score of 4.50. After four weeks of use, overall user satisfaction among older adults was at the highest level (Mean = 4.57, S.D.= 0.52). The highest-rated dimension was ease of use (Mean = 4.68), followed by the quality of recommendations received (Mean = 4.57), and overall satisfaction and willingness to recommend the system (Mean = 4.54). Furthermore, willingness to recommend the chatbot showed the strongest positive correlation with overall satisfaction (r = 0.537, p < .001). The findings indicate that the developed multi-agent chatbot demonstrated high system performance and was highly accepted by older adults, suggesting its potential as a digital tool for supporting holistic health care in community settings. Nevertheless, further development of response validation mechanisms is recommended, particularly to reduce hallucinations generated by the language model, together with longitudinal studies to examine sustained use and actual health outcomes among older adults.
