วัตถุประสงค์ในการทำวิจัยนี้ 1) เพื่อพัฒนาแอปพลิเคชันสำหรับติดตามการส่งข้อมูลสุขภาพผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง กรณีศึกษา ชุมชนเมืองเพชรบูรณ์ 2) เพื่อศึกษาความพึงพอใจต่อประสิทธิภาพการใช้งานแอปพลิเคชันสำหรับติดตามการส่งข้อมูลสุขภาพผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง กรณีศึกษา ชุมชนเมืองเพชรบูรณ์ กลุ่มตัวอย่างที่ศึกษา คือ แพทย์ พยาบาลวิชาชีพ นักวิชาการสาธารณสุขคลินิกหมอครอบครัว โรงพยาบาลเพชรบูรณ์ สาขาคลองศาลา จำนวน 6 คน และอาสาสมัครสาธารณสุข ชุมชนเมืองเพชรบูรณ์ จำนวน 45 คน โดยวิธีการคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง เลือกผู้ที่มีความรู้ความเข้าใจในการดูแลผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง เป็นผู้ป่วยหรือเป็นผู้ดูแลผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง การพัฒนาแอปพลิเคชันสำหรับติดตามการส่งข้อมูลสุขภาพผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง โดยใช้ขั้นตอนรูปแบบ SDLC (System Development Life Cycle) การประเมินผลด้วยการวัดความพึงพอใจต่อประสิทธิภาพการใช้งานแอปพลิเคชัน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา
 
ผลการวิจัย พบว่า แอปพลิเคชันสำหรับติดตามการส่งข้อมูลสุขภาพผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง ประกอบด้วย 2 ฟังก์ชัน คือ 1) ช่องทางติดตามการส่งข้อมูลสุขภาพผู้ป่วยกลุ่มโรคไม่ติดต่อเรื้อรัง 2) ช่องทางการบันทึกข้อมูลการปรับพฤติกรรมตามระดับความเสี่ยงทางคลินิกและผลการศึกษาความพึงพอใจต่อประสิทธิภาพการใช้งานแอปพลิเคชันจากกลุ่มตัวอย่างส่วนใหญ่มีความพึงพอใจภาพรวมอยู่ในระดับมาก ( x̄ = 4.05 , S.D. = 0.66)

Non-Communicable diseases Case study of Muang Phetchabun. The samples studied were : Doctors, Professional nurses and public health technical officer. Primary Care Cluster Phetchabun Hospital at KlongSala about 6 persons, the public health volunteers in Phetchabun Municipality about 45 persons. By selected of example group on specifically only and choose the knowledge man and insight to look and case the
Non-Communicable diseases or Patients or relatives, caregivers of patients with chronic Non-Communicable diseases. The develop Applications For the transmission of health information in Patient monitoring Non-Communicable diseases. By used the style of SDLC (System Development Life Cycle). The assessment of satisfaction of the performance on smartphone application. Data were analyzed using descriptive statistics.
 
The result of research can see that, Applications For the transmission of health information in Patient monitoring Non-Communicable diseases can know that 2 items functions. 1) To follow up on health information transmission of patients with chronic non-communicable diseases. 2) To recording of behavioral adjustments based on clinical risk levels. Satisfaction studied to use in the group of application of using with efficiency. The most of examples can make them to Satisfaction ( x̄ = 4.05, S.D. = 0.66).
